Endometriosis & Pelvic Floor Physical Therapy

If you have endometriosis and are still in pain, even after trying birth control, medications, or surgery, you are not alone.

At Cappuccino Physical Therapy, we specialize in helping women with endometriosis finally understand why they’re still hurting and how to get relief.

Why Endometriosis Causes Ongoing Pain (Even After Treatment)

Endometriosis is a complex condition where tissue similar to the uterine lining grows outside the uterus.

But here’s what most people are not told:

Endometriosis doesn’t just affect your organs—it changes your muscles, nerves, and movement patterns.

Over time, your body responds to pain by:

  • Tightening the pelvic floor

  • Creating areas of muscle tension in the abdomen, hips, and back

  • Developing guarding patterns that keep the nervous system on high alert

Even if the endometriosis is treated surgically, these patterns don’t automatically go away.

This is where pelvic floor physical therapy comes in.

Endometriosis Symptoms Are More Than “Just Painful Periods”

Endometriosis is often dismissed as “bad cramps”—but the reality is much more complex.

Many women go years without answers because their symptoms don’t fit the typical narrative.

Endometriosis can show up as:

  • Persistent pelvic pain (not just during your period)

  • Severe menstrual cramps that interfere with daily life

  • Pain with sex

  • Bloating or “endo belly”

  • Painful bowel movements or constipation

  • Urinary urgency, frequency, or discomfort

  • Low back, hip, or abdominal pain

  • Fatigue or feeling drained around your cycle

  • Pain with exercise or certain movements

These symptoms are SO common, but never normal

And importantly, many of these symptoms are influenced by the pelvic floor, muscles, and nervous system—not just the endometrial lesions themselves.

That’s why even when imaging is “normal” or after medical treatment, symptoms can persist.

How Pelvic Floor PT Helps Endometriosis

Pelvic floor physical therapy focuses on treating the secondary effects of endometriosis—the part that is often missed.

Research shows pelvic PT can help reduce pain, improve sexual function, and improve bowel and bladder symptoms

At our clinic, we focus on:

1. Reducing pelvic pain at the source

  • Release of trigger points and muscle tension

  • Nervous system downtraining

  • Decreasing sensitivity in the pelvic region

2. Restoring normal muscle function

  • Teaching your pelvic floor to relax (not just strengthen)

  • Improving coordination with breathing and movement

3. Improving painful symptoms like:

  • Painful periods

  • Pain with sex

  • Bloating and “endo belly”

  • Constipation or painful bowel movements

  • Urinary urgency or frequency

4. Addressing the whole body
Endometriosis doesn’t just live in the pelvis—we assess and treat:

  • Core and abdominal wall

  • Hips and low back

  • Posture and movement patterns

We’re Part of Your Endometriosis Care Team

Endometriosis is not a one-provider condition—it requires a team approach.

At Cappuccino PT, we help you navigate that.

We:

  • Help you determine if your symptoms may require further medical evaluation

  • Connect you with trusted excision surgeons when appropriate

  • Collaborate with your OB/GYN, fertility specialist, or pain management team

  • Support you before and after surgery to optimize recovery

Many of our patients come to us after being told “everything looks normal”—but they’re still in pain. We help bridge that gap.

What to Expect at Your Visits

Our sessions are one-on-one, 60 minutes, and fully customized to you.

Treatment may include:

  • Gentle manual therapy (internal and/or external—always with your consent)

  • Myofascial release and scar tissue work

  • Nervous system and breathing strategies

  • Movement retraining and exercise

  • Red Light Therapy

  • Education so you understand your body and your pain

You will never be rushed, dismissed, or told your pain is “just part of being a woman.”

Can Pelvic PT Cure Endometriosis?

No—and anyone who tells you that is not being honest.

Pelvic PT does not remove endometriosis lesions. Only excision surgery removes lesions and we HIGHLY recommend working with an excision specialist.

But it can:

  • Significantly reduce pain

  • Improve daily function

  • Help you return to exercise, intimacy, and normal life

  • Support better outcomes before and after surgery

You Don’t Have to Live Like This

If you’ve been told:

  • “Your scans are normal”

  • “This is just part of having a period”

  • “Try another birth control”

…but you still don’t feel right—

There is another layer to your pain that deserves to be addressed.
