      <image:title>Blog - The Pelvic Pulse - Pelvic Floor Physical Therapy for Overactive Bladder - The bladder is a hollow, muscular organ that stores urine produced by the kidneys until it’s ready to be released from the body. As the bladder slowly fills, it stretches, and stretch receptors in the bladder wall send a message to the brain, creating the sensation of needing to urinate.</image:title>
      <image:caption>The bladder can hold up to 500 milliliters of urine in women. Most people begin to feel the first urge to urinate when the bladder holds around 150 to 200 milliliters of urine. This urge is typically mild and can be ignored for some time. As the bladder continues to fill up to about 400 to 500 milliliters, that urge intensifies into a more urgent "I need to go right now" feeling. When it's time to urinate, the brain sends a signal to the bladder telling it to contract, which pushes the urine out. At the same time, the pelvic floor muscles, including the urethral sphincters, must relax to allow urine to flow. This coordinated effort between the bladder contracting and the pelvic floor relaxing is vital for a smooth, complete emptying. Ideally, the bladder will empty completely, and you will not feel the urge to urinate for a few hours. However, some issues can cause urine retention (meaning your bladder did not fully empty) or give you that sense of urgency even though your bladder is empty.</image:caption>

Tension in the pelvic floor muscles, especially around the urethra, can create pressure or irritation in the bladder wall. These include: Cystocele (Bladder Prolapse): The bladder bulges into the vaginal wall, caused by weakened anterior vaginal wall support. This is different from a rectal prolapse, in which your rectum is visible out of the anus.       <image:caption>Stage 1: The organ (e.g., bladder, uterus, or rectum) has moved slightly lower than its normal position into the vaginal canal. Typically, no symptoms are experienced, or they may be very mild, such as a slight feeling of pressure. Stage 2: The organ has descended further, approaching the opening of the vagina. Symptoms may become noticeable, including a feeling of heaviness or pressure in the pelvic area. Some women may experience mild urinary or bowel symptoms, such as difficulty emptying the bladder or rectum. Some may also notice a visual bulge internally.  Stage 3: The organ has descended to the level of the vaginal opening or may slightly protrude from it. Symptoms are often more pronounced, such as noticeable bulging, pelvic pressure, urinary incontinence, or difficulty with bowel movements. Stage 4: The organ has fully descended outside the vaginal opening. Symptoms are significant and may include a visible bulge, discomfort, pressure, difficulty with urination and bowel movements, and sexual dysfunction. This stage typically requires medical intervention, such as a pessary (more on pessaries shortly) or surgery, to relieve symptoms and restore support to the pelvic organs.</image:caption>
